If You Have One of the Qualifying Conditions, Get Pre-Qualified First
Step 1 – Email Us (info@mmpohio.com) the Following:
- Your medical documents that identify your qualifying condition. Only one page from your healthcare provider stating your condition is necessary.
- A copy of your Ohio photo ID (i.e. your drivers license).
- A completed copy of our New Patient Forms.
Step 2 – Document Review and Appointments
- Dr. Silver will review your documents to make sure you qualify for a card before we collect any money or set up an appointment.
- Please note, you will need access to an email account to complete your registration.
- Appointments can be done over the phone or at our office in Newbury, Ohio.
- Veterans and SSDI patients will need to provide proof of their status at the time of registration to receive a discount at dispensaries. Dr. Silver will explain the procedure during consultation.
Step 3 – Get Your Card
- Once you’ve completed your registration the card will appear on your device screen.
- After that, you will be able to download or print your card.

If you have medical documentation of any of the qualifying conditions you can be registered to receive your medical marijuana card the same day as your appointment.
