Get your medical marijuana card the same day as your appointmentAt Medical Marijuana Physicians of Ohio (MMPO) our office fees are for professional services that are required to qualify a person for the medical marijuana card. Our doctor’s office fee is for the examination that results in a medical card that is good for one year. No additional charge for follow ups during the year.

Patients will also be charged $50 by the Pharmacy Board at the time registration is complete in order to receive their card. A credit card is necessary for this step. Reduced Pharmacy fees are available for veterans and Social Security Disability recipients, which is half of the usual charge. But discounted fees are only available after patients send documentation of veteran or SSDI status to the Ohio Board of Pharmacy. This process of documenting discount eligibility delays getting your card until the Board of Pharmacy reviews your documents.

New Patient Forms

In order to fill out the forms below you need to download it to your computer and open it with Adobe Acrobat or any other program that reads PDF files. Please follow these instructions to ensure your information is accurately recorded.

  1. Click (right-click for most systems) on the “New Patient Forms” link below and download/copy/save as the file to your desktop or documents folder.
  2. Once opened you should see the fields highlighted where you can type in your information.
  3. When you’re done please select File > Save As and label the forms with your name.
  4. Attach the file to an email and send it to info@mmpohio.com.

If you have any problems print out the forms and fill them out by hand. Once complete you can mail the to our main office or fax them to us.
